CDOC TERM-LIMITED - Community Peer Support Manager (Social Worker/Counselor IV)

This is a TERM-LIMITED position and provides a support system that addresses the health and well-being of individuals exiting incarceration. This position is responsible for, but not limited to, the following:
- Coordinate pre-release meetings and introductions between incarcerated individuals and their post-release support teams for individuals who are eligible for medication-assisted treatment to a Regional Parole Office Peer Specialist, community recovery organizations, and other community resources six months before their release.
- Identify incarcerated individuals on MAT who are within 6 months of release.
- Facilitate the transfer of care and support plans from the correctional facility to Regional Parole Office Peer Specialist, community recovery organizations, and other relevant community-based resources. .
- Establish and maintain strong relationships with Regional Parole Office Peer Specialists, community recovery organizations, and other relevant community resources..
- Develop and implement a contingency management program in conjunction with the Division of Parole to incentivize engagement with peer recovery support services.
- Track progress and distribute incentives as individuals adhere to MAT protocols, and participating in recovery programs.
- Collect and report data on key performance indicators, including the number of individuals served, engagement with community resources, participation in the contingency management program, and rates of successful reintegration.
- Provide educational sessions on SUDs, harm reduction strategies, and MAT management to incarcerated individuals, and equip individuals with knowledge about community resources and how to access them effectively
- Maintain regular contact with individuals for a minimum of 90 days post-release to monitor progress.
- Participate in evaluation activities to assess the effectiveness of the CPSM role in improving outcomes for individuals on MAT.
- Maintain required credentials to include licensure and any certifications required for specialized behavioral health services.
Minimum Qualifications
- Applicant must possess a valid, active license from the State of Colorado or State participating in the Social Work Licensure Compact as a Licensed Professional Counselor, Licensed Clinical Social Worker, Licensed Psychologist or Licensed Marriage and Family Therapist.
- AND
- Applicant must have two (2) years of licensed or license candidacy experience, with current valid licensure from the State of Colorado or State participating in the Social Work Licensure Compact, to include Licensed Social Worker (LSW), Licensed Professional Counselor Candidate (LPCC), Licensed Clinical Social Worker Candidate (SWC) or Licensed Marriage and Family Therapist Candidate, and experience must be with the mental health and/or sex offender population.

- Advanced knowledge and a thorough understanding of Mental Health standards and guidelines;
- Ability to continuously expand knowledge base through continuing education of innovative clinical techniques and research;
- Strong interpersonal skills with the ability to collaborate effectively across facility programs and with external agencies;
- Strong leadership and analytical skills;
- Excellent written and verbal communication skills;
- Ability to read and comprehend the English language;
- Ability to effectively communicate fluently in English both verbally and in writing;
- Ability to create accessible documents and technical content that follows plain language guidelines with accessibility in mind.
- Ability to provide direct patient care to inmates;
- Must maintain required licensure.
