The Role:
As a Mental Health Care Coordinator, you’ll plan, manage, and implement care strategies for adults with mental illness. You’ll be a vital link between clients and the services they need, working closely with multidisciplinary teams and making regular community visits.
What We’re Looking For:
- MSW preferred, but BSWs with 3+ years of mental health experience are encouraged to apply
- Strong communication and problem-solving skills
- A genuine passion for helping others
- Must enjoy traveling within the Middletown area and surrounding areas
