QMHP/Case Manager Position
Permanent Supportive Housing Program
QMHP level Case Managers in this role provide psychosocial rehabilitative skills training to individuals who have experienced chronic homelessness and who are diagnosed with a disabling condition, including mental health diagnoses, who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective. Case managers operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams and cultivate a clinical skill-set by forging therapeutic relationships with diverse community members. Interventions and services are typically provided in the community and require the use of a personal automobile and cell phone.
Case Managers provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. Frequent service coordination with external and internal providers is necessary to support individuals in achieving independent functioning. Staff may provide all or some of the following: deliver therapeutic interventions fostering skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need. QMHP staff may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities.
