Job Responsibilities
JOB SUMMARY:
Location: The Living Bridge, 1498 Jesse Jewell Pkwy SE, Gainesville, GA 30501.
The Ryan White Program at the District 1-2 Public Health District provides outpatient medical and supportive services to uninsured and underinsured adults diagnosed with HIV/AIDS in North Georgia.
JOB DUTIES AND RESPONSIBILITIES:
Under broad supervision, provides professional nursing care in accordance with nursing protocols, policies, and/or procedures. The focus is on the prevention of illness, injury, or disability. This position promotes the health and maintenance of populations and cares for a population with special needs during disasters/emergencies, and biological and chemical protocols.
- Job includes significant amounts of critical thinking, coordination, and multi-tasking. The ideal candidate will know when to slow down, look patients in the eye and provide personalized service.
- Performs clinical duties for HIV-positive individuals with general supervision per protocol. Clinical duties include immunizations, STD screening and treatment, physicals, GYN examinations. Referrals are to be made for vision care, dental care, primary care, and mental health services.
- Consults with infectious disease and primary care physicians as needed to address the patients’ needs and concerns.
- Develops and updates Individual Service Plans and Acuity Scales at least every six months.
- Initiates and completes medical/psychosocial referrals.
- Enrolls and completes new and re-enrolling client intakes to care.
- Monitors daily clinic schedule for missed appointments and medical adherence (non-compliance) to ensure viral load suppression and continued linkage to care.
- Completes third party prior authorizations for medical payment.
- Reviews and completes Social Security HIV/AIDS Medical Verification Forms.
- Uses CAREWare and VHN (electronic health records system) to document all medical and supportive services.
- Assures all deliverables are met in accordance with the Ryan White Part B programmatic annexes and master agreements.
- Assists in gathering and completing programmatic reports and data.
- Participates in all clinic activities, quality management, consortium, and community initiatives.
- Attends job-related conferences and seminars.
- Performs other duties as assigned and necessary.
Minimum Qualifications
Bachelor’s degree in nursing from an accredited college or university AND One year of nursing experience AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification;
OR
Associate degree in nursing from an accredited college or university AND Two years of nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.
