Aging Life Care Manager

Aging Life Care Manager/Geriatric Care Manager-Work with interesting people, do something different almost every day, have time in the office, time traveling to clients wherever they are in the greater Tucson area (home visits, hospital, assisted living, skilled nursing facilities, medical appointments, etc.)  

TLC Coordination and Advocacy is a woman founded/owned, small local business providing services in high demand.

Minimum education required: Bachelor’s in social work, psychology, public health administration, nursing, or related fields. Unsure and interested? Let us know.

Basic office equipment operation (scanner/printer) knowledge or willingness to learn.

 

Job responsibilities:

Meet with clients and sometimes family members wherever they are (home, assisted living, retirement community, skilled nursing facility, or hospital)

Complete assessment of strengths, needs and available resources

Assist in identifying their priorities and needs

Assist in developing a care plan

Identify most beneficial resources for the identified needs

Investigate benefits and ensure services arrange are with covered providers

Make referrals to appropriate companies for identified needed services

Monitor services and troubleshoot if there are any issues

Coordinate and attend medical appointments, take accurate notes

Update other authorized medical providers, authorized family members, and any facility providing care

Ensure any new orders have follow up

Observe and report any new concerns to medical provider or family as appropriate

Advocate for clients wishes when appropriate or requested

Document any contacts with client, medical providers, family, or other providers including time spent

Ensure records are kept up-to-date

Assist with some insurance claims

Work independently and as a team member

Track mileage

Keep track of purchases and receipts

Learn care management software used

Take required trainings in timelines given

Help identify other useful trainings

Promote a positive public image

Must have reliable car and insurance (mileage paid)

Obtain care management certification within one year.

Job may involve some on-call duty after 6 months.
