Wrap Facilitator
Responsibilities include facilitating the formation and administration of a Wrap Around Service Coordination Team around each child, who is emotionally handicapped and/or at risk in the Lake and Porter County area. Educates clients and their families on the availability of community resources and assists them in accessing needed resources.
DUTIES & RESPONSIBILITIES:
- Assesses families’ strengths and needs, and will take lead in the development of the preliminary individualized service/education plan under the supervision of the Service Director and coordinates with the Wrap Around Group Supervisor. Assures all service providers in the wrap around plan are in place and complete the services they are instructed to perform in the time frames designated; reporting any non-compliance immediately to the program supervisor for action.
- Implements an initial assessment of the child and family within five calendar days of referral for a maximum of 10-12 families/cases. Provides a minimum of two face to face contacts with client/family per week. Completes CANS assessment and reassessments within specified time frames. Completes monthly team meeting with each family. Completes monthly report on each client’s progress within specified time frames. Completes IICP and reviews within specified time frames.
- Convenes a wrap around team and facilitates on going wrap around planning meetings with each family; making sure that the parent is the lead in selecting who is on the team and how they will support the child/family. Monitors the progress of each plan and coordinates delivery of service collaboration with community agencies as needed. This may include meetings at the school, probation, child welfare programs, medical/health clinics, etc. Assures a crisis plan is in place for each child and family team. Supports team members in developing a family centered, wrap around philosophy.
- Assures the ongoing integration of education and mental health services as developed in each child and family plan. Locates available resources and identifies gaps in local service delivery network. Supports families and team members in developing skills in advocacy, facilitation and assessing resources. Supports family in maintaining Medicaid or other insurance program for the child. Provides other case management services as needed. Provides skills training and development for both the client and the parent as needed.
- Provides treatment coordination and referral sources to other social services agencies, schools, courts and DCS. Therapists and families which may include, but are not limited to: linkage with needed resources, entitlement programs, financial, medical, mental health and other social services needs.
- Maintain appropriate documentation of records for Medicaid billing, assist in obtaining physician signatures in a timely manner, and consistent record keeping. Submits a monthly report of active clients, new client intakes and discharges/transfers for the month to the supervisor and to our financial department for billing purposes.
- Participate in meetings, training, staffing as required. These will not be limited to Edgewater Systems but will also include required attendance at Geminus meetings.
- Submits, as required, all appropriate records regarding staff activity in a timely manner. Submits, as required, all appropriate incident reports and crisis situations to supervisors at Edgewater Systems and Geminus Corporation as well as the referral source within a 24 hour period of time from the event.

- Always maintain a minimum of the company’s productivity requirement (100%) by providing case management and/or therapy services as needed.
Qualifications
ESSENTIAL JOB REQUIREMENTS:
- Involves frequent contact with personnel of other Center programs particularly medical services.
- Knowledge of community resources and community systems.
- Frequent contact with DCS, court probation, Hammond, East Chicago and Gary School systems.
- Maintain confidentiality and privacy of all data, records, forms, reports, procedures and any other information regarding client relations for Edgewater Health
- Responsible for maintaining good client relations in all forms of communication not limited to written, oral and body language.
- Follows all agency policies as outlined
POSITION REQUIREMENTS:
- Education – Bachelor degree in in Psychology, Social Work or other related human service field
- Experience – Minimum of 2-year experience preferred working in assessment and coordination of services with SED children, adolescents, and their families.

Other - Possess a valid driver’s license.

- Furnish own inspected, automobile as needed for Center work and transportation of clients.
- A good driving records.
- Appropriate automobile insurance and valid license tags.
- Maintain a vehicle free of mechanical problems and carry personal vehicle insurance.
