An exciting opportunity to help develop, design, and implement a new Community Membership Program aimed at increasing meaningful community inclusion and quality of life of individuals served!

 

Established in 1959, Uplift, Inc. has been providing community services for almost 65 years. Uplift was established by local families seeking educational options for their children at a time when children with IDD were not allowed in public schools. Uplift now provides a variety of services including residential, employment, case management to low-income individual experiencing IDD. Uplift strives to continue to provide quality support services and develop new innovative services like Community Membership to meet the needs of individuals served. Our mission is to create conditions under which individuals with IDD experience; presence and participation in the community, respect and dignity, personal choice, opportunities to develop competence and to create and maintain personal relationships.

Coordinating with Director of Clinical Services, Director of Residential Services, and Residential Administrator to develop, design and implement a Community Membership Program for individuals with IDD. Responsibilities include developing policies and procedures adhering to program description and state licensing requirements, developing workflows for accepting referrals, getting authorizations for services, service implementation plans, coordinating with internal and external providers for Personal Care Plans (PCP) , developing necessary paperwork for scheduling, data collection and reporting outcomes, finding and developing networking relationships with community resources, developing a thorough data base of local service connected and non- service connect resources for employment, volunteering, transportation, social and recreational activities, and networking opportunities. Perform a Plan Do Study Act evaluation to measure the success of the service and any needed improvements.
