Mental Health Facilitator

Do you have four years of experience working with youth and families in a mental health setting? Join our Wraparound Team as a Mental Health Facilitator!  

 

As the Mental Health Facilitator on the team, you will be responsible for meeting with clients and caregivers on a weekly basis, provide psychoeducation, and further treatment goals by coordinating care between providers. The Mental Health Facilitator will be the team clinical leader for the respective caseload; as a result, you will have the opportunity to consult with team members on clinical and case issues.

 

*Must be eligible for MHRS certification (4 years of clinical experience in a mental health setting to apply)*

 

ABOUT WRAPAROUND

The Solano Wraparound program serves Solano County children and adolescents in the Child Welfare, Probation and Mental Health systems that are transitioning from or are at risk of going to out of home care or juvenile hall. The Wraparound approach focuses on supporting children by enhancing natural resources within their families and communities so that they can live in the least restrictive, most family-like setting possible. Services are primarily held in the community: in families’ homes, group homes, foster homes, schools, etc.

Responsibilities

  • Develop Child and Family teams made up of natural supports for the purpose of treatment planning and sustainable support
  • Cultivate and maintain engagement and alignment with community members, county partners and other MH providers
  • Identify Family Finding and engagement needs for each youth and family as needed and work with the team members to build connections for family
  • Intervene with the clients’ behavior management programs as needed
  • Chart mental health notes, treatment plans, outcome measures, incident reports, and other documentation accurately and in a timely manner
  • Participate in 24 hours on call support line system
  • Participate in individual and/or group supervision

Qualifications

REQUIRED

  • Bachelor’s degree in Counseling/Psychology/Social Work
  • Must be eligible for MHRS certification (4 years of experience  providing services in a mental health setting)
  • Flexible schedule with the ability to work some evenings hours as needed
  • Be part of a rotating emergency on-call system, scheduled ahead of time
  • Must be at least 21 years of age
  • Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policy
  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
  • Proof of up-to-date COVID-19 vaccination or an HR-approved medical/religious exemption

PREFERRED

  • Bilingual in Spanish
  • Experience working with children in out-of-home care
  • Ability to work as a team player, with diverse population and demonstrates cultural competency

SKILLS OF AN IDEAL CANDIDATE

  • Ability to develop Child and Family teams composed of natural supports for treatment planning and sustainable support
  • Skills in cultivating and maintaining relationships with community members, county partners, and other mental health providers
  • Ability to complete documentation promptly and efficiently

Schedule

  • Full-time
  • Flexible schedule with the ability to work some evening hours as needed
  • Be part of a rotating emergency pager system, scheduled with supervisor ahead of time

Benefits

  • Starting at $31.12 - $33.62 per hour, commensurate with experience
    • As of July 7th, this starting range will increase to $32.05 - $34.55 per hour
    • Additional compensation provided upon passing bilingual language proficiency exam
