MSW - Social Worker - Behavioral Health - ATLANTA, GEORGIA

Peachford Hospital is searching for a Master's (MSW) prepared Case Manager to work full-time in our Social Services Department. Must be willing and able to work across all patients population – ages 4 and above (children, teens, adult men and women and geriatrics).

Peachford Hospital, located in Atlanta, Georgia,  is a 246-bed, acute care psychiatric hospital and has been providing compassionate care since 1973. Peachford features individual units for children, adolescents, adults, and seniors, and offers inpatient acute care, partial hospitalization, and intensive outpatient programs. Nearly 10,000 patients receive the psychiatric, addictive disease and behavioral health care they need at Peachford every year.

Case Managers at Peachford are part of an interdisciplinary treatment team charged with providing quality therapeutic and case management services to our patients from admissions to discharge. Case Managers are empowered to assess their patient’s needs using their own personal style, while delivering superior quality care. Case Managers are responsible for completing the following duties and tasks:

  • Conduct psychosocial assessments to document patient history and identify preliminary issues for treatment focus by interviewing the patient and his/her family members.
  • Provide individual, group and family solutions-focused interventions using appropriate therapeutic techniques and models. Facilitate process-oriented and didactic groups.
  • Document treatment and case management provided to patient in the patient’s electronic medical record.
  • Plan, lead and document group therapy sessions with assigned patients.
  • Develop and coordinate an individualized discharge plan for the patient by utilizing information obtained from the patient, treatment team and medical record to determine discharge planning needs. Identify available appropriate family or community resources such as group homes, halfway houses or treatment providers.
