Overview: 

You will provide professional nursing services to hospice patients or supervise the provision of nursing services aimed towards enrolling hospice patients in accordance with accepted standards of practice and guidelines. You may perform nursing services in a variety of care settings and/or roles, including but not limited to case management, admissions, or on-call.

How you will make an impact:

  • You will complete a comprehensive and initial and ongoing assessment of:
    • Patient and family to determine hospice needs
    • The impact of the terminal diagnosis on the patient’s physical, functional, psychosocial grief, cultural and spiritual implications, and verbal non-verbal communication patterns
  • You will assist in developing, implanting, evaluating, and updating the plan of care and make recommendations plan revisions as needed
  • You will work with the interdisciplinary team to prioritize and provide patient and family-oriented hospice services. 
  • You will provide end-of-life care including comfort care and symptom management according to NorthStar Care Community indicators

What you need:

  • Graduation from an accredited school of nursing with possession of a current license to practice as a registered professional nurse in the State of Michigan.
  •  Minimum of one (1) to two (2) years of varied nursing experience required.
