Bachelors Level Clinical Coordinator

Position Summary: The Bachelor Level Clinical Coordinator has the general responsibility of coordinating care and triage of clients to the most appropriate level of care. Duties performed include but are not limited to referring for determination, handling insurance information, gathering relevant information for clinicians, maintaining patient information, discharge planning and coordination of aftercare. Bachelor’s level Clinical Coordinator must demonstrate excellent customer service and have enthusiasm about the program and the Pine Rest organization. 

Principal Duties and Responsibilities

• Completes the triage of clients to establish the appropriate level and coordination of care, to include but not limited to, referring for determination, handling insurance information, gathering information for clinicians and/or psychiatrist, maintaining patient information, discharge planning and coordination of community resources. 

• Communicates with other involved clinicians, and other human service organizations treating client to ensure continuity of care, coordination of services, and excellent patient experience. 

• Serves as a liaison between community resources, payers, family, patient/client, and hospital- or clinic-based providers of care. 

• Responsible to document and report all necessary information according to established policies and procedures when applicable. 

• Contributes to Service Excellence by following quality improvement standards, maintaining professionalism, initiating and maintaining positive relationships with departments, co-workers and external customers. 

• Maintains supervision as required. 

• May routinely contact appropriate third-party payers or community mental health authorities for required preadmission screening or pre-authorization for Behavioral Health admission. 

• May enter patient information into Electronic Medical Record.

• May refer patients to appropriate outpatient, hospital, or community services as necessary.

• Must maintain annual training and demonstrate competency in the required programmatic training. 

• Performs other duties or functions as assigned. 

Additional Principal Duties and Responsibilities within HRS: 

• Professionally and calmly handles crisis calls, responding appropriately to the acuity and needs of internal and external customers. 

• Establishes therapeutic relationships with assigned patients/clients including engaging, deescalating, screening, and managing crises. 

• Works in various clinical settings with diverse patient/client populations with complex needs. 

• Participates regularly in departmental and staff meetings. Serves on programs or organizational committees as required and as time and workload allow. 

• May screen calls and direct callers to appropriate community resources. 

• May complete phone triage, set up admission time, and notifies licensed professional staff. 

• May complete resource and discharge planning for patients including but not limited to scheduling aftercare appointments, arranging placement/transportation, developing a plan for access to medications and communicating with external providers regarding resources. 

Additional Principal Duties and Responsibilities within ORS: 

• Acts as liaison between psychiatric program services and collaborating hospitals external to Pine Rest, including but not limited to the Emergency Department, Medical Surgical nursing units, and hospital owned outpatient medical clinics. 

• Presents behavioral health intake information to the psychiatrist in an organized concise formulation of the problem including impression and recommendations. 

• Contribute to the development of metrics, data tracking and evaluation tools designed to assess community needs. 

• Provides prompt care navigation to hospital emergency departments with patients in psychiatric crisis. 

• Performs principal duties and responsibilities via video technology, primarily during hours 12pm-11pm 

• Responsible to answer calls forwarded from Consultation Liaison services line 

• Responds to calls from EAP line. Obtains necessary information to schedule appointments and connect caller with master’s level clinician if crisis care is needed

Required/Desired Qualifications: 

Education, Training, and Experience

• Bachelor’s degree in Social Work, Psychology, or related field required. 

• A minimum of one year of related work experience related to coordinator of patient services preferred.

• Ability to provide culturally inclusive care with a wide range of individuals in a diverse community setting. 

• Interpersonal relationship skills necessary in order to relate to a wide variety of internal and external contacts including patients and families, multi-disciplinary treatment team members, other hospital employees, and a variety of individuals in the social sphere of the patient under treatment. 

• Familiarity with pre-admission utilization review procedures, and case management of individuals in psychiatric crisis. 

• Familiarity with electronic medical records system. 

• Verbal and written communication skills which enable the worker to convey clear, thorough, necessary and timely information to individuals, family, colleagues, supervisory and management personnel. 

• Requires emotional stamina to deal with physical, emotional, social, and spiritual needs of patients. 

• Moderate decision-making skills are needed in order to process workflow. Critical thinking skills necessary in order to contribute to decision on discharge planning needs. 

• Demonstrate strong organizational skills and attention to detail. 

• ORS specific – o Knowledge of the resources and policies of community mental health. o Willingness and ability to utilize video technology as primary mode of service delivery.
