SUD Navigator

POSITION SUMMARY:

As part of an integrated and comprehensive community mental health team serving individuals with severe and persistent mental illness, the Substance Use Disorder (SUD) Navigator provides social support, community resource referrals, advocacy for patients and their family and crisis management through a comprehensive approach to care management. This position also provides direct services to consumers, including individual, group and family therapy. Duties include providing skill-building support, assisting patients in setting personal goals, and recovery principles. There is ongoing follow-up, basic motivation interviewing, and goal setting with patients. Provides therapy for patients with behavioral health problems complicated by environment and personal difficulties. The individual is fully engaged and integrated as part of the patient care team, providing input into care plans, and alerting the medical staff of psycho-social issues complicating health conditions.

MINIMUM JOB REQUIREMENTS:

  • Bachelor’s degree in related field.
  • Registration or certification from a recognized certifying organization, agency or Board evidencing training and expertise in areas appropriate to SUD services (CAADAC, CCAPP, CADTP, CADDE, CARR)
  • Minimum 2 years of experience providing SUD services in a community and/or outpatient setting

COMPENSATION:

$37,960 to $51,950 (Full-Time/Union)
