Make a Difference: Nurse Practitioner for Community Care Clinic

Sentara Community Care was created with the intent to invent new and unique models of care that strive to not only address but aid in alleviating some of the socially determined health disparity in our under-resourced communities. Since 2022, this program has expanded rapidly in practice and in location. We are looking for a seasoned Family Nurse Practitioner who shares in our passion and can help us supply high quality medical care and support to communities that may otherwise not have easy or financial access to medical care.

Position Highlights Per Location in Virginia:

• Richmond: Primary care support for maternal, infant, pediatric, and adult health. Comfortable providing some basic prenatal care
• Harrisonburg: Primary care with ample pediatric experience, for infants, pediatrics, adolescents, and adults. 
• Halifax: Maternal health focus with primary care support for infant, pediatrics, and adults. Will complete services in Halifax, Danville and Lynchburg.
• Newport News: Primary care for pediatrics, adolescents, and adults.

Position Highlights for all Locations:

• Minimum of 3 years’ experience as a licensed provider treating family medicine, pediatrics, and adults with complex issues
• Full-time position with non-traditional hours of noon to 6:00pm or 7:00pm, plus 1-2 Saturdays per month 
• Shared call schedule, with RN first call
• Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients’ health holistically. 
• Review quality and outcome metrics monthly with the healthcare team
• Collaborate with local civic and faith-based leaders on community education programs
• Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice
