Case Manager - Adult Mental Illness

$2,000 Sign On Bonus after 90 days of employment!

NeMCMHA is a certified HRSA Loan Repayment site – you may qualify for up to $50,000 in loan forgiveness with a 2-year commitment.

 

As an Adult Case Manager at Northeast Michigan Community Mental Health (NeMCMHA), you will provide goal oriented and individualized supports to individuals that focus on improving self-sufficiency for the individual through: assessment, planning, linking, advocacy, monitoring, and coordination of activities as identified in their person centered plan. You will collaborate with other staff and the individual to provide services in the least restrictive environment, providing support and intervention as needed. Your background in mental health and case management, as well as your passion for people will serve you well in this role.

Hours – Full-Time hours are generally Monday-Friday 8am-5pm with a rotational on-call.

Must have – Bachelor’s degree in Social Work or be a Registered Nurse, licensure according to state guidelines (RN, LLBSW/LBSW). A valid driver’s license is also required.

Salary – Bachelor’s: $21.40 - $29.50 | Registered Nurse $26.38 - $36.43 | PLUS! $2,000 Sign-On Bonus and Additional Compensation from Rotational On-Call

 

What is Adult Case Management?

Case management assists individuals in gaining access to needed health and dental services, financial assistance, housing, employment, education, social services, and other services developed through the person-centered planning process. 

Targeted case management services are available for all adults with serious mental illness, those with co-occurring substance use disorders who have multiple service needs, have a high level of vulnerability, require access to a continuum of mental health services, and/or are unable to independently access and sustain involvement with needed services.
