About the Job:

Provides culturally appropriate and unique individual group, and family counseling for clients with mental health and AODA issues as well as other comprehensive services. This position will be responsible for working with the Prevention Department to assist in the clinical guidance and counseling for community members of all ages.

Here’s what you’ll be doing:

  • Focuses within the context of cultural and spiritual values and needs of the individual or family system, developing community-based relationships and evidenced-based programming and policy with a focus on recovery and prevention that meet the cultural and spiritual needs of the Forest County Potawatomi Community.
  • Collaborates with other professionals and community resources to manage client intakes, assessments, and treatment planning
  • Provides biopsychosocial-spiritual assessment of clients, mental health and AODA therapy to individuals, groups, and families, including children and adolescents
  • Manages a case load of clients, making referrals when appropriate to community resources to assist in recovery from AODA, mental health, or physical illness and to provide access to services such as financial assistance, work programs or education
  • Evaluates and identifies environmental impediments to client progress through interviews and review of client records, ensuring communication and understanding about care between client, family and health care team
  • Researches additional or alternative resources; providing and maintaining educational materials and knowledge of resources available
  • Provides outreach and behavioral health education for the Forest County Potawatomi Community
  • Coordinates inpatient treatment when determined appropriate
  • Audits case files routinely to ensure compliance with all regulatory and licensing agencies and maintains accurate and complete, timely, and accurate documentation without judgment
  • Participates in prevention activities, support groups, classes and other initiatives

Here’s what you’ll need to be successful:

  • Graduate degree in Social Work (APSW) or current Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC); current AODA certification or ability to obtain certification within 1 year
  • Experience in working with persons in crisis in behavioral health and AODA inpatient/outpatient settings
  • Experience with a diverse population with general behavioral health issues, serious emotional disorders, and AODA issues. Other significant and applicable experience will be considered.
  • Knowledge of Forest County Potawatomi culture or willingness to gain said knowledge.
  • Must possess exceptional communication and organizational skills, be able to customarily and regularly exercise discretion and independent judgment in significant matters, and be able to relate with empathy and sensitivity to staff, clients and community
  • Must be proficient with the use of an Electronic Health Record, clinical documentation and basic computer systems such as MS Office applications
  • Valid driver’s license
  • (Health Only) FCP complies with all CMS Mandates & all Health Division employees must receive the COVID-19 vaccine prior to hire. Additional immunization requirements may also be necessary.
