We are the community organization responsible for the delivery of mental health, substance abuse treatment coordination, and developmental disabilities services for Kent County. We need individuals who are passionate about making a difference by helping others.
We are seeking a Supports Coordinator responsible for Person-Centered Planning and to develop an Individual Plan of Service. Additionally, to provide on-going linking, coordinating, and monitoring of services.
The person-centered planning process and development of individual plan of service requires meeting and communicating regularly with persons with intellectual/developmental disabilities, community providers, guardians, and/or family members, independent schedule management, meeting of deadlines, and creation of documentation into an electronic health record daily. Individuals who are computer savvy, organized, and self-motivated will be able to excel in this position.
This is a regularly scheduled position, Monday to Friday, from 8 am to 5 pm. No weekends or holidays. A flexible hybrid working arrangement is available for this position. The employee may perform work from a home office or work site during regular business hours.

MINIMUM QUALIFICATIONS:
- Bachelor’s degree in human services field (Social Work, Education, Psychology, Sociology, Counseling, Community Mental Health, etc.)
- One year of experience treating or working with a person who has intellectual disability or specialized training received through fieldwork and/or internships.
- Possession of a valid driver’s license and access to reliable transportation for job related use
- Strong organizational skills, with experience in person-centered planning
- Knowledge of Culture of Gentleness
- Demonstrated understanding of persons with developmental disabilities
- Demonstrated communications skills that effectively convey information
- Demonstrated active listening, i.e. listening to what other people say and asking appropriate questions
- Demonstrated time management skills
PREFERRED QUALIFICATION:
- Licensed BSW
- Lived experiences with mental illness/developmental disabilities/substance use disorders valued
- Knowledge of trauma-informed theories, principles, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions).
