Medical Social Worker

Job Responsibilities

*This position does not require any on-call hours and offers a flexible work schedule upon successful completion of 6-month probationary period. The job responsibilities allow for a combination of remote/hybrid (remote/office) with required attendance at in-person meetings as well as other designated in office hours. Selected candidates who have been approved to work hybrid must complete that work within Michigan. Hours are Monday through Friday, 8:00 AM to 5:00 PM. No weekends, holidays, or nights. Occasional special events may require evening or weekend participation.*

Children’s Special Health Care Services (CSHCS) strives to enable individuals with special health care needs to have improved health outcomes and an enhanced quality of life through the appropriate use of the CSHCS system of care.

Kent County Health Department is currently seeking a full-time Medical Social Worker for our Children’s Special Health Care Services program. The successful candidate will work as a member of a multidisciplinary team providing client advocacy and client care management for families of children with special health care needs. This individual will be responsible for community resource navigation, support group planning, development of patient care plans as well as the occasional home visit to assist with service provision, working with the community to bridge program service, and participation in monthly support groups and community events.

Minimum Qualifications

Bachelor’s Degree in Social Work and two years of experience providing care coordination/casework services to individuals/families.
 
Required educational preparation and licensure: LLBSW, LBSW, LLMSW, or LMSW.

Ability to positively interact with individuals and/or families from diverse cultural and socio-economic backgrounds required. Must possess a valid, unrestricted Michigan driver's license and have access to transportation in the performance of job duties. Must have the ability to work with electronic medical records. Ability to speak Spanish is highly desirable.
