Sawyer County Public Health Department - VISTA Member
The VISTA member’s service will improve upon the resources the county lacks in its most recent community health needs assessment which includes mental health, substance misuse, and chronic disease. The VISTA will improve population health by creating community health promotion initiatives among the population of Sawyer County, Wisconsin, with an emphasis in underserved areas of high poverty providing and creating resources to those in poverty that cannot afford proper services.
The VISTAs propose is to engage and collaborate with the Sawyer County Healthier Together Steering Committee on how to address community needs shown in the community health needs assessment.
- Engage with Sawyer County Healthier Together Steering Committee and research and develop a plan to address issues address in the community health needs assessment.
- Research and develop a implementation plan that prioritized initiatives.
- Establish relationships and create groups for the community health improvement plan for Sawyer County Healthier Together. Groups will address either mental health, substance misuse, and chronic disease.
- Develop a sustainability plan for future VISTA members
Research and secure funding opportunities to implement community health initiatives prioritized by the steering committee to improve the health of those persons living in poverty.
- Research requests for applications for applicable and available grants that align with serving people living in poverty.
- Develop letter of intent and application for submission that can be used currently or in the future.
- Research and engage with new partners utilizing the created community groups to establish funding opportunities that align with the sites mission.
- VISTA will develop a continuation plan/guide for future VISTA members to carry out project.
Research and develop plan to roll out recruitment of volunteers for initiatives prioritized by the Sawyer County Healthier Together steering committee to increase the health of the community
- Engage with recruitment team to research and develop volunteer descriptions to share opportunities.
- Communicate with existing networks and community resources to research best recruitment strategies for recruiting volunteers.
- Collaborate with new volunteers recruited to begin implementation for initiatives that were designed.
- Develop a sustainability plan for future VISTA members
