HealthNet of Rock County - VISTA Member
The VISTA Member activities will increase access to healthcare to the low income and uninsured population by evaluating processes, studying population health metrics, and laying the groundwork of integral components to ensure sustainability of funding, volunteer staffing, medications and the delivery of high-quality healthcare to the most vulnerable populations. Through the VISTAs service patients can be seen at no cost which can keep patients out of poverty by not receiving unaffordable medical bills.
Engage with data collection and data reporting for grants, organizational metrics, or public awareness to research clinics quality measures.
- Collaborate with clinic managers to ensure that HealthNet of Rock County is continuously meeting Wisconsin Association Free and Charitable Clinic's Gold Seal of Excellence.
- Begin evaluating and tracking the results of patient surveys in medical, dental, and behavioral health clinic to further resources for clinic patients.
- Research and develop an action plan for HealthNet of Rock County to achieve NCQA Medical Home Accreditation.
- Develop a sustainability plan for future VISTA members.
Research partnerships for HealthNet of Rock County to secure pharmaceuticals to be given to uninsured patients
- Coordinate with a digital patient assistance program to ensure HealthNet of Rock County will obtain medications that improves the clinic in fulfilling new prescriptions and refills in its medication repository.
- VISTA will be creating a network of providers who order samples from pharmaceutical companies that provide further access to necessary medications.
- VISTA will develop a continuation plan/guide for future VISTA members to carry out project.
Improve quality metrics for HealthNet of Rock County, Inc. through participation in standards and QA/QI committee participation.
- Monitor and implement the objectives of the Quality Assurance and Quality Improvement Committee, and create the Quality Assurance and Quality Improvement Plan for 2023-2025.
- Monitor and implement interventions and data reporting for the 2021-2023 Quality Assurance and Quality Improvement Plan.
- Create and publish metric dashboard on all HealthNet public channels to share and promote information.
