Medical Social Worker -Maternal Infant Health Program

Job Responsibilities
*This position does not require any on-call hours and offers a flexible work schedule upon successful completion of 6 month probationary period. The job responsibilities allow for a combination of working from home and office.*

Kent County Health Department is currently seeking a full-time Medical Social Worker. The successful candidate will work as a member of a multidisciplinary team providing home and virtual visits to pregnant women and infants. This individual will be responsible for completing a comprehensive assessment of service needs to develop a plan of care for clients, determining appropriate referrals for families and follow-up to evaluate effectiveness of services. Duties will also include documentation according to program guidelines.

Minimum Qualifications
Master's degree in Social Work and one year of experience providing care coordination services to individuals/families. Current State of Michigan LMSW required.
 
 Ability to positively interact with individuals and/or families from diverse cultural and socio-economic backgrounds required. Must possess a valid, unrestricted Michigan driver's license and have access to transportation in the performance of job duties. Must have the ability to work with electronic medical records. Ability to speak Spanish is highly desirable.
