2022 - 2023 Health Care Network Development Associate VISTA

As a VISTA Member at Health Care Network clinic in Racine, WI you will gain professional experience by working to help HCN provide high quality comprehensive health care to those in need with a focus on expanding access to healthcare for those without insurance or who are under-insured by: helping to build and maintain partnerships with schools, coalitions and other community organizations, developing clinic policies and procedures, growing HCN’s volunteer pool, grant-writing/fundraising, helping maintain the Standards of Excellence, continuing to develop HCN’s outreach via social media and more.

Goal of the Project: The mission of the Wisconsin Association of Free & Charitable Clinics (WAFCC) is to support, strengthen, and advocate for the uniqueness of the Wisconsin free and charitable clinics, the patients they serve, and the communities with whom they partner. The location for this project is Health Care Network (HCN) in Racine, WI. Racine is on Lake Michigan and near Milwaukee and Chicago. The mission of Health Care Network is to provide free or low cost health and dental care to low income, uninsured residents of Racine County, WI. Over 380 volunteer providers provide this care. Health Care Network is the only free clinic in Racine County and one of the largest in the State of Wisconsin. In operation since 1987, the clinic is in need of a deep review of clinic policies and procedures with updates as well as a training plan to communicate the updates. The VISTA Member’s charge is: 1. Review and update clinic and volunteer policy manuals 2. Gather continuous data on patient statistics and analyze and determine at risk populations. 3. Monitor patients enrolled in Self Monitoring Blood Pressure (SMBP) program, working to improve communication process between staff and reporting patients. 4. Assist program director with managing student volunteers within the clinic. 5. Develop a diabetic care program focusing on team-based care and Glooko technology. Increasing organizational capacity to meet the expanding need for health care services in the community is imperative to the sustainability of HCN and WAFCC.
