Healthcare Specialist III

We’re looking for a Healthcare Specialist III to join our team as a Bridges Liaison who will work in Huerfano & Las Animas Counties - with the 3rd Judicial Court system.
What You’d Be Doing
As a Healthcare Specialist III you will work as a Bridges Liaison to provide clients referred for competency restoration services with referral information for providers, assist them with connection and enrollment in restoration services, and keep timely data on client progress in competency restoration services. The Bridges Liaison will be responsible maintaining connections and knowledge of community providers and resources to assist them in screening and coordinating of referrals into treatment for individuals needing competency restoration and other supportive services. Under the administrative and clinical supervision of the Crestone Supervisor the Bridges Liaison is responsible for.
Specifically, the Healthcare Specialist III
  • Organizing services in coordination with the Program Supervisor/Court Administrator.
  • Coordinating clients from Huerfano and Las Animas Counties into competency restoration services and other supportive services.
  • Identifying stressors and precursors to crises and assisting clients in overcoming barriers to receiving services.
  • Coordinating competency restoration services between providers, the judicial system, and other appropriate agencies.
  • Providing education regarding Mental Health, Substance Use, co-occurring diagnoses, Intellectual and Developmental Disabilities (IDD) and competency restoration services to members of the criminal justice system where appropriate.
  • Managing and maintain all required data points for the state offices reporting on a consistent a thorough manner.
  • Providing a consistent support and availability until the client has achieved a higher level of functioning and released by the court.
  • Linking the clients with community resources as needed.
  • As needed assist in arranging transportation for scheduled appointments.
  • Providing support to reduce usage of/divert from more restrictive treatment settings or jail.
  • Keeping accurate, complete and up-to-date records with 100% compliance with company standards.
  • Maintaining competency and proficiency with all data collection and record keeping platforms.
  • Performing other job related responsibilities as assigned.

What We’re Looking For—The Must-Haves
  • Baccalaureate degree in social work, psychology, or a closely related field
  • Experience in case management/community systems navigation, particularly with the criminal justice and/or mental health systems Experience working with people with substance use and co-occurring disorders
  • Experience working with individuals with substance use/co-occurring disorders services to include those with Intellectual and Developmental Delays (IDD) and experience working with persons diagnosed with severe & persistent mental illness is required
  • Exceptional flexibility, time management, and documentation skills
  • Must be a self-starter and self-motivator
  • Experience working with a multi-disciplinary team and comfort working with members of judicial teams
  • Valid Colorado driver’s license
  • Access to reliable transportation during work hours
