SIS Assessor
SIS Assessor: Allegan County Community Mental Health (ACCMHS) is seeking a candidate for a full time (40 hours per week) Support Intensity Scale (SIS) Assessor. The candidate will schedule and complete a standardized assessment with adults that have a developmental or intellectual disability. The resulting report is utilized by providing agencies, case managers and supports coordinators to advocate and enhance the Person Centered Planning Process. A SIS takes from 1.5 to 2.5 hours to complete and is a guided conversation respecting the individuals unique support needs.
Responsibilities:
- Conducts a SIS assessment for each individual receiving services in conjunction with the Person Centered Planning process.
- Provides interpretation and analysis of SIS results.
- Maintains a minimum reliability status of “Qualified” in IRQRs conducted by an AAIDD recognized SIS Trainer.
- Provides consultation on the development of the Individual Plan of Service (IPOS) that incorporates the results of the SIS assessment.
- Conducts SIS interviews with two or more respondents (may include self-advocates) that are familiar with the individual receiving services.
- Comply with all applicable regulations and requirements (HIPAA, Medicaid, etc.)
- Record data in SIS online databases
Qualifications:
- Bachelor’s Degree in human services, healthcare, or social work
- At least one year of experience working with persons with disabilities
- Qualified Intellectual Disability Professional (QIDP) preferred but not required
- A commitment to longevity in the developmental disabilities system and the SIS program
- Excellent communications skills and computer
- Must successfully pass SIS Certifications for adults and children upon hire.
