Rehabilitation Case Coordinator

Rehabilitation Case Coordinator-Full Time-35-40 hours per week-Lansing based non-profit agency seeks a full time Rehabilitation Case Coordinator for contract work with the State of Michigan. The Rehabilitation Case Coordinator position is a full-time (35-40 hours per week) non-exempt position with a starting wage of $19.50 per hour, benefits available following 90 days of successful employment. This position is not a Civil Service position.
Essential functions of position include:
Analyze rehabilitation needs of disability populations and of individual customers. Develop Individual Plan for Employment (IPE), which outlines the services the individual will receive through MRS or community agencies to address their disability related barriers and to assist them with gaining employment. Provide vocational counseling and guidance. Develop community resources. Draft authorizations for expenditures for services. Provide or refer for job placement, job development and post-employment services. Manage case service budgets and records.
Individual tasks related to this position may include:
•             Gathers pertinent educational, social, medical, psychological, and vocational data relative to the customer; determines customer’s needs and eligibility for rehabilitation services.
•             Determines vocational objective and develops vocational rehabilitation plan with customer.
•             Provides vocational counseling and guidance.
•             Coordinates community services and authorizes expenditures to help customer reach vocational objective.
•             Provides job development services for placement-ready customers, including job-seeking skills training and developing job leads.
•             Maintains assigned employer accounts to create employment opportunities for customers.
•             Provides post-employment services necessary to maintain suitable employment for customers.
•             Manages case service budgets and case records with established policies and procedures.
•             Performs related work as assigned.
Minimum Hiring Requirements
1.  Bachelor’s Degree in Human Service, Social Work or closely related field; enrolled in a closely related Master’s program preferred. 
2.  A minimum of two years’ human service experience.
3.  Personal experience with a disability and/or knowledge of disability related issues.
4.  Must successfully pass a criminal background check, sex offender registry, and DHHS Central Registry. 
5.  Must have access to reliable transportation as position may require travel.
6.  Occasional evenings and weekends.
7.  Strong verbal and written communication skills; Computer skills, including Microsoft Word, Outlook, Excel and data entry.
Position is not a Civil Service position. It will be a purchased service provider position, which means you would be a contract worker of the State of Michigan.  There are some limitations within these positions in doing case work in the case management system, however, you would have a counselor or administrative staff member available to assist in these situations. There may be both part and full-time positions available.
