Case Manager: Allegan County Community Mental Health is seeking a full time Case Manager to work within a team environment. The candidate will be responsible for a Person Centered Planning approach to provide supports, linking to appropriate community resources, coordination and monitoring of services.
Minimum Qualifications
  • Bachelor’s degree in human services field (required)
  • Licensed as a psychologist, social worker, or a licensed or limited-licensed professional counselor.  
  • One year experience working with adults with a mental illness.
  • Strong organizational skills, with experience in person centered planning
  • Valid Michigan driver’s license
  • Ability to work well in varied settings, maintain good relationships with consumers, and community professionals. Position requires flexible hours, and strength-based approaches.
Responsibilities:
Provide Case Management Services to individuals on assigned case load in the form of:
  • Inter-agency case management including: Social Security Benefits: SSI, SSDI, PASS, etc; FIA Benefits: Home Help, Medicaid, Food stamps, etc., Courts,
  • Transportation for clients as required by treatment plan. Provide documentation to the agency of appropriate license and good driving record.  This may involve verification through the use of a Motor Vehicle Record.
  • The development of Natural Supports: family, friends, religious organizations/leaders, neighbors, teachers, community people of any and all involvement/contact, etc.
  • Intra-agency case management with: Personal Supports, Residential, Respite, Career Concepts/Employment, Psychiatric Services, Clinical Therapies, Consumer Loans, Substance Abuse, Transportation, ACT, etc.
  • Act as a consultant to individuals and their support systems.
  • Gather and exchange relevant information with health care professionals and ACCMH psychiatrist/clinicians.
  • Complete required paperwork within the standards set forth by MDCH, Medicaid, CARF, Quality Assurance, and the affiliation.
  • Monitor support providers in the provision of services and the implementation of person centered plans.
  • Participate in clinical case conferences, staff development (in-services, conferences, professional organizations) and agency committees as assigned
