Judson Center is a premier non-profit human services agency which provides compassionate and comprehensive services to children and families in need in SE Michigan/Metro Detroit. We provide services to over 8,000 consumers annually in four Centers of Excellence: Autism, Behavioral & Mental Health, Adoption and Foster Care and Vocational Disability Services. We operate out of five regional offices in SE Michigan.

General Program Description
Judson Center Outpatient Services offers traditional and crisis driven therapy interventions to children, adolescents, and adults who need support with managing their emotional, behavioral, and/or mental illnesses that are impacting their quality of life.

Job Summary
A case manager provides comprehensive case management services for adults with serious mental illness or families with children who have serious emotional and behavioral issues to improve their quality of life and maintain the highest possible ability to function within the community.
 
Primary Duties and Responsibilities
The Outpatient Case Manager will:
  1. Provides supportive activities and skill-based interventions to children, adolescents, adults and families to assist in meeting goals.
  2. Performs advocacy work with schools, courts and other social service agencies as needed.
  3. Connects families to community resources.
  4. Collaborates with collateral professionals, referral sources, and providers of ancillary services on behalf of consumers.
  5. Completes documentation in accordance with contractual requirements and agency guidelines. This includes progress notes, collateral contacts and provision of after care.
  6. Obtains prior authorizations for services in a timely manner as required.
  7. Meets or exceeds case load and productivity benchmarks.
  8. Completes all mandatory trainings, as required by contract, Judson Center, and licensure.
  9. Attends all scheduled meetings, including supervisions, program meeting, clinical and all staff.
  10. Performs other duties as assigned.

Job Qualifications
  1. Clinical experience, knowledge of best practices, evidence based practice models, trauma informed care, and co-occurring mental health and substance use disorders preferred.
  2. Knowledge of crisis interventions, community resource and solution focused systems approaches.
  3. Minimum three years’ experience providing mental health services to families and children or State licensure in Social Work and a minimum two years’ experience providing mental health services to families and children.
  4. Demonstrated competency in computer skills using electronic health record and Microsoft Office.
  5. Ability to comply with all applicable licensing rules, accreditation standards, code of ethics, insurance reimbursement rules, and Judson Center’s policies and procedures.
  6. Ability to provide culturally competent services that shows sensitivity to the service population’s cultural and socioeconomic characteristics.

Education, Certificates, Licenses, Registrations
  1. Bachelor’s degree in Human Services is required, social work preferred.
  2. State licensure in Social Work
  3. Valid Michigan Driver’s License, appropriate insurance and use of own vehicle.

Working Conditions
  1. Non-Traditional work hours in the office, family homes and community
  2. Job travel, utilizing personal vehicle with mileage reimbursement
  3. Working with consumers with a variety of behaviors, including aggression

Physical Requirements
While performing the duties of this job, the employee is regularly required to, walk, stand, sit, communicate verbally and in writing, hear, and use hands and fingers to operate a computer and telephone keyboard. Close vision requirements due to computer work. Regular, predictable attendance is required. The employee has the potential to encounter unpredictable and aggressive consumers. The position has mostly an inside work environment and occasional outside work with exposure to weather conditions.
