Assistant Nurse Manager, Home Based Primary Care

Assistant Nurse Manager (ANM) is a position who works in collaboration with the Nurse Manager and functions as a supervisor. The ANM assumes accountability for the coordination of home care visits and the communication between Veterans, staff members, and VAMC providers. The ANM will be expected to coordinate his/her schedule to provide coverage for the HBPC Nurse Manager.
The Assistant Nurse Manager is a registered professional nurse accountable to the Home Based Primary Care (1-113PC) Nurse Manager. In collaboration with the Nurse Manager, functions as a supervisor and assumes accountability for the coordination of home and community based services and programs within HBPC for the Saginaw VAMC. Duties included, but not limited to:
• Requires the nurse to have recent knowledge, experience, and demonstrate competence in HBPC.
• Executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines and influence organizational mission and health care.
• Provides communication between Veterans, HBPC staff, VAMC providers and community resources.
• Must demonstrate leadership in delivering and improving holistic care to the adult and geriatric population through collaborative strategies and in compliance with American Nursing Association Standards, in addition to Medical Center and Department standards.
• Maintains current knowledge of practice standards and guidelines related to Home Based Primary care.
• Promotes continuity of care through developing an individualized plan of care in collaboration with the patient, caregivers and the interdisciplinary team.
• Provides therapies and treatments based on Nursing assessment, LIP orders, protocols. Basic cardiac life support protocols, standards of care and in compliance with facility references.
• Assesses patient and family learning needs, instructs patient and family on self-care. disease prevention, health promotion, considering issues related to patient's age and cultural diversity concerns related to religion, ethnic background or other issues.

Physical Requirements: Physical demands include repetitive body movements including the following: frequent reaching, standing, walking. and lifting of equipment and/or patients to provide patient care.
This position requires extension of the hand(s) and arm(s) in any direction including above or below shoulder level,during the provision of patient care, including retrieval of needed equipment, supplies and to make contact with the patient.
This position requires the maintenance of an upright position to perform duties associated with patient care. This position is typically non-sedentary, and frequently involves being in the standing position. This position requires moving about on feet to accomplish tasks. such as assisting with patient ambulation and moving one patient area to the next. This position requires the raising of objects from a lower position to a higher position or the movement of objects horizontally position to position. Lifting may be done to move a patient from one location to the next. and in the movement of equipment or supplies. This position requires exerting up to 50 pounds of force occasionally and/or
up to 25 pounds of force frequently to move objects. This position requires writing or otherwise working with fingers and for hands, including one or both hands frequently. This is done when taking a patient's pulse, utilizing a stethoscope, administering medications, entering information into the computer. etc.
Note: Must have Basic Life Support (BLS) certification and Advanced Cardiac Life Support (ACLS) prior to assuming clinical duties. The BLS requirement is satisfied by successfully completing an American Heart Association (AHA) BLS Healthcare Provider Course or Military Training Network (MTN). The ACLS requirement is satisfied by attaining, and maintaining, current status through an approved AHA Advanced Cardiac Life Support Course. BLS courses are normally offered within 1-5 days of entry on duty. No online certifications will be accepted. Selected applicants may be required to complete an online onboarding process.
Qualifications
Basic Requirements:
(1) Citizenship. Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.)
(2) Graduation from School of Nursing
(a) Graduate of a school of professional nursing approved by the appropriate State agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: ( NOTE: See exception in subparagraph (b) below for candidates who are enrolled in a MSN Bridge Program)
1. The National League for Nursing Accrediting Commission (NLNAC), an accrediting arm of the National League for Nursing located at 61 Broadway, Floor, New York, New York 10006 or call (800) 669-1656 extension 153. The NLNAC accredits all levels of nursing programs. Additional information may be obtained from the NLNAC web site; or
2. The Commission on Collegiate Nursing Education (CCNE), an accrediting arm of the American Association of Colleges of Nursing (AACN). The CCNE accredits bachelors and masters degree programs, and is located at One Dupont Circle N.W., Suite 530, Washington, DC 20036 or call (202) 463-6930. Additional information may be obtained from the CCNE web site.
(b) The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. In these programs, students are given a certificate of professional nursing to sit for the NCLEX and should submit this certification to VA prior to appointment. Students do not earn a BSN, but receive a MSN degree upon completion of course work. A copy of the MSN transcript must be provided to VA. Bridge programs that confer a master's degree also fully meet the education requirement, even though a Bachelor's Degree is not awarded.
• (c)] In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration (see paragraph 2a(3)) will meet the requirement of graduation from an approved school of professional nursing.
3) Registration
(a) Condition of Employment. A registered nurse (RN) will have a current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e.,Puerto Rico) of the U.S. or in the District of Columbia. The appointing official may waive this registration if the RN is to serve in a country other than the U.S. and the RN has registration in that country (e.g., Philippines). The RN must maintain a current, full, active and unrestricted registration to continue employment with VA.
(b) Impaired Registration. An impaired registration is any registration(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status. A registered nurse who has or ever had any such impairment to their registration as listed above may be appointed only in accordance with the provisions of chapter 3, section B, paragraph 15 of this part.
English Language Proficiency. RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
• Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
• Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
• Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
• Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
• Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.
