Bi-lingual ACE Case Manager

RESPONSIBILITIES: Under the supervision of the Program Manager, the Case Manager is responsible for providing services to 
COMPACT Achievement Center youth 11-19 years old and their families. Associates are responsible for working with the team in 
developing team goals and objectives and in meeting those goals and objectives, including but not limited to:
DUTIES:
• Receive and process participant referrals
• Schedule and conduct intake and assessment appointments with participants and families
• Develop strength-based, individualized family treatment plans including goals and objectives.
• Provide family-centered case management services, and other services related to program goals and objectives.
• Conduct home visits, school visits, field visits, in addition to office visits to best meet the needs of the youth and family.
• Maintain case records and client files 
• Participate in Multi-Disciplinary Team meetings with other team members.
• Appropriate coordination of contacts with other public and private organizations and individuals, as necessary to effectively
serve clients. (e.g. progress reports to referral sources, client referrals to other agencies).
• Accurate manual and digital data entry
• Build rapport with diverse case load of participants
• Monthly statistical reports of services provided as required by COMPACT.
• Outreach to target populations to effectively reach those in need of services.
• Participation in special projects and activities as related to agency’s goals and objectives.
• Participation in agency special events, such as fundraisers, annual meetings, tabling events and open houses.
• Assist in the planning, coordination, development, monitoring, and evaluation of services for a youth and family with 
emphasis on quality of care, and continuity of services.
• Participation and assistance in implementing program components.
• Provide follow-up case management services to program graduates
• Oversee and provide tutoring services to participants on a daily basis
• Assist in the transporting of youth to and from program in company
