RN Home Care Case Manager
Location: Western & Central Ohio
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Weekend Rotation: Every 4th weekend (Friday & Monday off after worked weekend)
Holiday Rotation: 2 holidays per year
Pay: $35.00 – $40.00/hour (based on education, experience, and certifications)
Hours: Full-time (40/week)
Sign-On Bonus: $5,000 (for Full-Time position)
About the Role
EverHeart Hospice is seeking a compassionate RN Home Care Case Manager to provide skilled, patient-centered care in patients’ homes. You’ll support patients and families across routine homecare and end-of-life care while collaborating with an interdisciplinary team.
Key Responsibilities
- Provide skilled nursing care, symptom management, education, and emotional support
- Complete home visits, admissions, and ongoing assessments
- Coordinate care with physicians, IDG team members, and community partners
- Manage documentation, DME coordination, and physician orders
- Provide independent evening, weekend, and holiday coverage according to the agency’s rotation and backup schedules.
- Educate patients and families on hospice and palliative care services, coverage, and appropriate interventions.
- Coordinate referrals, admissions, and care across home, hospital, and nursing facilities.

Qualifications
- Current Ohio RN license
- Minimum 2 years RN experience (Med/Surg, Oncology, or Community Health preferred; hospice a plus)
- Valid driver’s license and reliable transportation
- Strong communication and time management skills
- Ability to work independently
