Position Summary:
HMHD is seeking a licensed Physical Therapist to join our Rehabilitation Services team. The Physical Therapist evaluates, treats, and provides instruction to patients to prevent, correct, or alleviate physical disabilities and pain caused by injury, disease, or deformities. Treatment modalities may include therapeutic exercises, massage, hydrotherapy, energy-based treatments, and functional training.
This position requires adherence to physician-established plans of care, Texas State licensure requirements, and HMHD policies. Candidates must be prepared to work extended hours as necessary to meet patient needs.
Responsibilities:
- Perform initial patient evaluations within 72 hours of referral and develop individualized treatment plans.
- Implement and adjust physical therapy plans of care in collaboration with physicians, patients, and families.
- Document patient progress, daily notes, and discharge summaries accurately and in a timely manner.
- Direct and supervise Physical Therapist Assistants and Rehabilitation Aides.
- Conduct patient instruction, including exercises, functional training, and pain management techniques.
- Participate in discharge planning and performance improvement initiatives.
- Maintain compliance with HIPAA, corporate integrity, and HMHD Code of Conduct.
- Provide ongoing education and training for staff, students, and community groups.
- Attend staff meetings, in-services, and continuing education programs as required.
Minimum Qualifications:
- Graduate of an approved physical therapy program.
- Current licensure to practice physical therapy in the state of Texas.
- CPR certification.
- Effective verbal and written communication skills.
- Ability to meet physical requirements, including standing, walking, bending, lifting, and patient transfers.
