The State of Connecticut, Department of Public Health (DPH) is currently seeking a qualified Dietitian/Nutritionist, Clinical Social Worker, Occupational Therapist, Physical Therapist, or Speech Language Pathologist for a Health Facility Surveyor position in the Facility Licensing and Investigations Section (FLIS). The Facility Licensing and Investigations Section is part of the Healthcare Quality and Safety Branch of the Connecticut Department of Public Health.
Please note: Candidates who are offered and accept a position with DPH Facility Licensing and Investigations Section must be free from conflicts of interest at the time of appointment and continuously thereafter in accordance with the CT Code of Ethics and complete all required trainings.
POSITION HIGHLIGHTS:
- Location: 410 Capitol Avenue, Hartford, CT
- Schedule: Typically Monday - Friday
- Shift: Full-time, First Shift
- Hours: 35 hours per week
- This position will be required to travel and some off hours may be required
- There may be an opportunity for partial telework and a flexible/alternate schedule in accordance with State of Connecticut Policies

IN THIS ROLE YOU WILL:
- Investigate complaints and unusual occurrences
- Conduct in-person visits to health care facilities to conduct the survey process;
- Observe, assess, and evaluate quality of care, quality of life, and resident rights;
- Prepare inspection reports, narrative reports, documentation for informal conferences and formal hearings relative to findings of inspections or investigations;
- Perform monitoring duties relative to non-compliant providers, monitoring of emergent situations in health care institutions and strike monitoring;
- Provide information on regulatory requirements;
- Other duties as required.
