Mental Health Caseworker
The Qualified Mental Health Professional (QMHP) position is responsible for providing behavioral health services, reentry planning, and support to individuals incarcerated in the Titus County Jail as part of the RISE Titus Jail-Based Behavioral Health Program. This is a grant-funded position supported through the Mental Health Grant for Justice-Involved Individuals (MHGJII) and is currently funded through August 31, 2028.
Responsibilities include:
- Conduct behavioral health screenings and assessments to identify mental health and substance use needs.
- Provide psychoeducational groups focused on mental health, coping skills, substance use, and relapse prevention.
- Assist with development and coordination of individualized reentry and discharge plans.
- Coordinate referrals and linkage to behavioral health treatment, housing, and community support services.
- Collaborate with jail staff, internal program staff, and community providers to support continuity of care.
- Complete daily documentation of services in accordance with agency, HHSC, and program requirements.
- Maintain compliance with safety, confidentiality, and operational procedures within the jail setting.
- Serve as a member of the interdisciplinary team and participate in staffing and care coordination as needed.
- Support individuals in accessing services following release to promote stability and reduce recidivism.
MINIMUM QUALIFICATIONS: Bachelor’s degree in Psychology, Social Work, Counseling, or a related behavioral health field. Must meet criteria as a Qualified Mental Health Professional (QMHP) under Texas Administrative Code. Minimum of one year experience in behavioral health, case management, or social services. Valid Texas driver’s license and reliable transportation. Ability to pass required background checks and obtain clearance to work within the jail facility.
PREFERRED QUALIFICATIONS: Experience working with justice-involved individuals or within a correctional setting. Experience conducting behavioral health assessments, facilitating groups, and discharge planning. Knowledge of community resources related to behavioral health treatment, housing, and reentry services. Training in trauma-informed care and motivational interviewing. Strong communication, organization, and collaboration skills.
