State Hospital North is seeking a highly organized, collaborative, and mission‑driven Resources & Services Navigator focused on Discharge Coordination to support safe, timely, and effective transitions of care for our patients. This role is essential in ensuring continuity of services, coordinating with internal and external partners, and advocating for patient needs throughout the discharge process.
State Hospital North in Orofino, Idaho, is a licensed 60-bed psychiatric hospital providing primarily involuntary inpatient treatment for adults in psychiatric crisis who are committed to the Department of Health and Welfare. State Hospital North collaborates with patients, their families, and the referring regional behavioral health centers to develop targeted goals and objectives for the individual’s treatment episode while simultaneously focusing on critical supports and arrangements for follow-up care after discharge.
The Idaho Department of Health & Welfare is a drug-free workplace. EXAMPLE OF DUTIES:
Care Coordination & Collaboration
- Coordinate with external service delivery systems—including Magellan—to support and organize hospital admissions and discharges.
- Maintain strong communication with internal hospital departments such as Medical Providers, Nursing Leadership, Clinicians, Pharmacy, Dietary, and the Assistant Hospital Administrator Clinical regarding discharge timelines, needs, and details.
- Work closely with the Admission Coordinator to schedule admissions and discharges based on priority, bed availability, and patient needs.
Discharge Planning & Patient Advocacy
- Support the development of discharge plans and provide input to the Treatment Team.
- Perform and coordinate ongoing discharge activities, including entitlement applications, patient advocacy, and placement arrangements.
- Coordinate follow‑up medical appointments, including medication management, ensuring they occur within required timeframes.
Medicaid, Long‑Term Care, and Benefits Coordination
- Work with Idaho’s Medicaid Eligibility Unit to ensure patient applications are completed prior to discharge, requiring extensive knowledge of Medicaid laws, rules, and verification processes.
- Collaborate with long‑term care units regarding nursing homes, pre-admission screening and resident review, certified family homes, group homes, and other specialized placements.
Placement & Community Resource Coordination
- Maintain up‑to‑date knowledge of community‑based placement options including HART homes, Certified Family Homes, ResHab, and other resources.
- Maintain communication with service providers regarding recently discharged patients, ensuring continuity of care and addressing needs such as medications, belongings, or additional information.
MINIMUM QUALIFICATIONS:
You must possess all the minimum qualifications below to pass the exam for this position. Please make sure your resume or work history supports your meeting the minimum qualifications for this position. Failure to do this may disqualify you from being considered for this position. It is highly recommended to attach a one-page cover letter to your application to demonstrate how you meet the requirements below.
- Good knowledge of human services principles and practices. This is typically gained by a bachelor’s degree or higher in a human services field (social work, education, nursing, psychology, counseling, sociology, etc.) OR at least three years of work experience as a human services professional (e.g. social worker, counselor, rehabilitation specialist, etc.) assisting individuals and families with accessing community resources including benefits and services.
- Experience in case management. This is typically gained by at least one year of professional case management experience that included case plan development and monitoring.
Below is preferred experience. It is not required for the position but applicants with this experience may receive consideration over other applicants.
Experience in discharge planning, admissions, patient care coordination. Typically gained by at least six months of relevant work experience.
