The Southeastern Mental Health Authority (SMHA), under the umbrella of the State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is seeking an experienced and mission-drive Advanced Clinical Social Worker (PCN 26210) to join and help lead our Wellness Recovery Advocacy Program (WRAP) team in Norwich.
This is a unique opportunity for a skilled clinician to make a measurable difference in the lives of adults with significant mental health needs while contributing leadership to a high‑functioning, multidisciplinary community-based team.

- First shift, 8:00 a.m. - 4:30 p.m.

WHAT YOU WILL DO:
As a key member of the WRAP Team, you will provide both direct clinical services and clinical leadership that supports client stability, independence, and recovery.
Your responsibilities will include:
- Providing clinical oversight and consultation to WRAP team members in partnership with the Clinical Supervisor and the Supervising Clinician
- Conducting clinical assessments including Functional Assessments, Risk Assessments, Quarterly Reassessments, Recovery Plans, and Discharge Summaries
- Delivering crisis assessment, intervention, and safety planning
- Supporting clients with significant mental health needs — many with extensive treatment histories, co-occurring medical conditions, and risk of hospitalization or homelessness
- Overseeing the teaching of independent living skills in areas such as home management, money management, health and wellness, transportation, and daily living routines
- Facilitating individual and group psychoeducational and skill‑building sessions
- Assisting clients in maintaining entitlements (SSI/SSDI/DSS benefits) and navigating complex medical, court, housing, and legal systems
- Collaborating with hospitals, residential programs, probation/parole, courts, community providers, conservators, and representative payees
- Providing services primarily in the community, meeting clients where they are and fostering independence in natural settings
- Ensuring timely, accurate documentation that meets agency, regulatory, and revenue requirements

The Southeastern Mental Health Authority is a community mental health agency whose primary goal is to provide high quality behavioral health services to those in our community with mental health and substance use disorders who are 18 years or older and reside in New London County and its surrounding towns. Services, which include case management, outpatient behavioral health, crisis intervention, respite care, and forensic services, are recovery oriented and offered in a trauma informed and culturally sensitive manner. We support individuals to identify and achieve their own goals in recovery, fostering independence, dignity, and ability. As a State Operated Local Mental Health Authority, SMHA is responsible for administering all DMHAS contract funded behavioral health services in Southeastern Connecticut.
The Wellness Recovery Advocacy Program (WRAP) team is a recovery-oriented, multi-disciplinary Community Support Program with a multidisciplinary clinical team. The program provides intensive, rehabilitative community support, crisis intervention, individual/group psycho-education, peer support, medication management, linkages to other community-based providers/services, coordination of services/care, and individualized skill building in activities of daily living in order to maximize each individual’s level of independence and functioning. The majority of services are provided to clients in the community and are personalized to meet the clients’ expressed needs/preferences.

MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE
One (1) year of experience as a licensed clinical social worker.
PREFERRED QUALIFICATIONS
- Experience working in community-based settings with clients diagnosed with severe and persistent mental illness and substance abuse disorders.
- Experience teaching daily living skills, such as money management, food management, home management, and accessing transportation with clients in the community.
- Experience working with clients in accessing community resources and in securing and maintaining housing.
- Experience working with clients who are pre-contemplative and contemplative in their recovery.
- Experience completing initial assessments for patients/clients with acute psychiatric and acute addictive problems.
- Experience working on a multi-disciplinary community-based team/program.
- Experience working with clients who are undergoing severe psychological distress and/or exhibiting behavioral dysregulation.
- Experience completing such documentation as Recovery Plans, Reassessments, DLA-20s, Periodic Assessments, WITS notes, Releases of Information, Social Security Applications, and entitlement redeterminations.
SPECIAL REQUIREMENTS
- Incumbents in this class must possess and retain a license to practice clinical social work pursuant to Sec. 20-195 (m-z) of the Connecticut General Statutes.
- Incumbents in this class must possess and maintain eligibility for participation in federal health care programs as defined in 42 U.S. § 1320a-7b(f).
