RNAC Registered Nurse Asessment Coordinator
Overview:
Responsible for the daily operation and supervision of the RNACS and Restorative Aides, including management of the Resident Assessment Instrument (RAI) Minimum Data Set (MDS) process including but not limited to ensuring the accurate and timely completion and electronic submission of the required data; auditing documentation to ensure accuracy of the data submitted;
coordination of the Interdisciplinary Team for development and implementation of care plans based on data collected (Care Area Assessment or CAAs) and submitted; assessment of residents' conditions to ensure proper documentation and Medicare/Medical Assistance benefit utilization. Also responsible for tracking and reporting information on all facility residents for performance improvement and fiscal accountability. Duties will be performed according to the highest ethical standards and in adherence to the Corporate/Facility Compliance Plan.
Responsibilities:
- RAI Coordination:
- Responsible for the coordination, encoding of data and electronic submission of Medicare/Medicaid/OBRA mandated MDS/RUGs (Resource Utilization Groups) and other related documents as Federal and State required.
- Maintains and keeps current calendar for RAI compliance.
- Ensures completion of all resident assessments within the time frames mandated by State and Federal Regulations.
- Coordinates interdisciplinary team assessments to ensure time lines and accuracy.
- Coordinates and satisfies documentation of MDS by interdisciplinary team.
- Ensures electronic data submission within the timeframes dictated by State and Federal Regulations.
- Performs correction procedure of MDS assessments according to regulation.
- Reassesses resident on an ongoing basis.
- Ensures that timelines for collecting and transmitting data for all types of MDS assessments will be consistent with State and Federal Regulations.
- Completes revisions to care plan as necessary for optimal resident care.
- Promptly notifies interdisciplinary team when a change of condition RAI is initiated.
- Monitors and ensures accurate and appropriate documentation for the MDS assessment and notifies Director of Nursing of concerns or omissions.
- Communicates daily with interdisciplinary team to monitor resident needs.
- Provides feedback to the interdisciplinary team regarding assessments and documentation.
- Maintains and revises skilled resident list and attends weekly interdisciplinary meeting to address skilled needs/discharge plans.
- Coordinates MDS schedule with therapy program manager, or designee, to ensure correct ARD/assessment type is scheduled.
- Identifies residents who demonstrate a significant change in status and coordinates assessments.
- Fiscal Accountability:
- Monitors all skilled services to ensure accurate and efficient benefit utilization.
- Reports all RUGs scores to Executive Management.
- Monitors and reports Case Mix Index (CMI) scores per procedure.
- Codes and sequences clinical data such as diseases, procedures and therapies using the International Classification of Diseases and other appropriate coding systems. (Current version is the 9th revision, Clinical Modification ICD-9-CM.)
- Responsible for assisting RNAC’s, billing, and other personnel with the ICD-9-CM coding and the accuracy of that coding.
- Performs periodic coding quality reviewed.
- Provides accurate, regular and timely data to the billing department necessary for billing functions, especially Medicare status, Diagnoses/ICD-9CM/ICD-10 Codes and RUG categories.
- Oversees all certifications/re-certifications for skilled services as required by regulation.
- Ensures that Medicare coverage is discontinued when the Federal criteria is no longer met.
- Assists in the preparation of the Department budget.
- Monitors financial performance and adjusts day to day operations to meet expense and revenue goals while maintaining standards of quality customer service.
- Quality Improvement Accountability:
- Monitors to track accuracy of documentation for MDS assessments.
- Monitors assessment completions of each discipline.
- Consistently reports on opportunities to initiate improvement processes and participates in facility Continuous Quality Improvement (CQI) process.
- Prints Quality Measure Reports and ensures appropriate Process Improvement procedures are implemented as needed.
- Obtains, prints as necessary, and monitors all related reports, including but not limited to Monthly Statistical (MDS Activity, New Admission, Discharge without Return, Changes to Identifiers) Facility Quality Measure Profile, Verification Report and report of Denials/Errors/Warnings, and communicates information to DON.
- Direction and Education of Staff:
- Performs duties incidental to the position as delegated by the DON.
- Completes payroll activities for RNACs, LPNAC’s and Restorative Aides.
- Supervises the coordination of care provided by the Restorative Aides.
- Oversees implementation and ongoing review of Restorative Nursing Program.
- Ensures appropriateness of selected restorative nursing programs for residents participating and adjust plan of care as needed.
- Provides regular formal and informal educational in-services to the interdisciplinary team on assessment, documentation and care plan development.
- Participates in the orientation of new personnel related to the RAI process.
- Ensures that all members of interdisciplinary team are taught how to document to ensure proper reimbursement at appropriate level.
- Attendance of all mandatory meetings.
- Supervisory Responsibilities:
- Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
- Performs other duties as assigned.
Qualifications:
- Current, active Pa. Registered Nurse license, Associate's degree (A. A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; preference of 2 or more years experience in a similar position; or equivalent combination of education and experience.
- Graduate of approved school of nursing and current registration as a RN in Pennsylvania.
- Working knowledge of computers, Microsoft Word and Excel required.
