Family Nurse Practitioner

Position:  Family Nurse Practitioner for In-Home Health Assessments

Job Status:  PRN

Location:  Ashtabula, OH

Pay:  $95/Per Completed Assessment + $2,000 signing bonus

 

POSITION SUMMARY:

The Nurse Practitioner (NP) brings expert care to members, through clinical assessments, screenings, and education. This role may provide care in a variety of settings including member/participant homes, assisted living facilities, telehealth visits, and skilled nursing facilities.


ESSENTIAL JOB FUNCTIONS:

• Perform screenings including medical history, physical exam, medication 
reconciliation and education.
• Effectively identify, evaluate, and address the concerns of the member, employee, or participant.
• Order and interpret diagnostic imaging, laboratory results and provide clinically
appropriate interventions, additional tests, or referral as needed.
• Administer and order point of care and lab-based testing.
• Refer members to PCP, case management, disease management programs, and/or appropriate specialist as necessary.
• Work collaboratively with a variety of stakeholders including risk & quality teams, staffing, operations, physicians, case managers, social workers, care givers and ancillary medical personnel.
• Participate in all mandatory compliance and other training programs, seek guidance for compliance-related concerns and adhere to all applicable laws, regulations and Matrix policies and procedures.
• Create and maintain clear documentation according to clinical standards, Matrix guidelines and protocols.
• Maintains positive control of all PHI information and compliance with OSHA and State Board of Nursing.
• May act as a preceptor for new providers.
• Other duties may be assigned.

 

POSITION REQUIREMENTS:

Educational Requirements:
• Bachelor of Science in Nursing preferred.
• Master’s Degree required OR commensurate experience and satisfactory completion of NP licensure.
• Current RN and NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority.
• Board certification by AANP, AACN or ANCC, in Adult, Family Practice Geriatrics or Adult Gerontology Primary Care, or ACNPC-AG (Adult Gero) Acute Care NP, or ACNPC (Adult) Acute Care NP.
• Current CPR certification.
• Multistate licensure preferred.
 

Knowledge, Skills, and Abilities:
• Demonstrated ability to:
o Perform comprehensive geriatric, adult physical exams, based on scope of 
practice for licensure and certification.
o Identify, diagnose and document acute and chronic disease.
o Effectively reconcile medications.
o Diagnose using evidence based clinical practice.
o Recognize conditions, characteristics, and behaviors that place members at risk.
o Identify and document the needs of the clinically complex.
• Possess a valid state driver license and proof of coverage and meet the company’s driving and automobile liability insurance standards.
• Ability to travel to assigned locations via public transportation including airplanes.
• Proficient computer skills, including e-mail and Microsoft Office.
• Effective written communication skills including but not limited to writing reports and correspondence.
• Strong verbal communication and effective listening skills, including the ability to present and maintain a professional, empathetic demeanor at all times.
• Demonstrated ability to prioritize work, meet deadlines, and use time efficiently.

 

Physical and Psychological Demands:
• Requires sitting, bending, stooping, twisting, kneeling, crouching, crawling and/or stretching 
from seated or standing positions.
• Requires eye-hand coordination and manual dexterity sufficient to operate medical 
equipment, keyboard, telephone, copier, calculator, and other office equipment.
• Requires close vision, peripheral vision, and ability to adjust visual focus, hearing and smelling.
• Must be able to communicate information via telephone or computer.
• Requires moderate to intense concentration due to the complexity of the job.
• Must be able to lift and/or move up to 25 lbs.
