Position Overview

The Division of Developmental Disabilities is seeking to hire a Social Worker II for the Shapiro Center located in Kankakee, Illinois to perform professional social work services in the treatment of individuals with intellectual disabilities on an assigned unit.  These services include assessment treatment (individual and group therapy), general case management and discharge planning.  Verifies compliance with Joint Commission, Centers for Medicare & Medicaid Services (CMMS) and other agency goals, standards and plans confirming good clinical care and individual safety.  Serves as a member of the interdisciplinary team for an assigned caseload.  Ensures compliance with facility, department, and Public Health policies and procedures.

 

Essential Functions

  • Performs professional social work services in the treatment of individuals with intellectual disabilities on an assigned unit.
  • Provides case management, including completing funding applications and acting as liaison between the Shapiro Center and community agencies.
  • Develops discharge plans, with the interdisciplinary team, individual and family, according to the individuals clinical and social needs assessment and treatment process and in collaboration with community agencies. 
  • Participates in facility team meetings on a daily, weekly, semi-monthly and monthly basis.
  • Prepares reports, forms, schedules, and correspondence accurately and in a timely manner.
  • Assists family/guardian with understanding financial information regarding services provided, obtaining guardianship, knowing their responsibilities as guardian, and informing family/guardian of services provided at Shapiro Center and services provided by community agencies in conjunction with Shapiro Center.
  • Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.

Minimum Qualifications

  • Requires a master’s degree in social work from a recognized college or university supplemented by one (1) year of professional social work experience in the evaluation and treatment/habilitation of mentally ill, developmentally disabled, or other disabled persons.

Conditions of Employment

  • Requires the ability to physically restrain individuals as necessary to prevent injury to individual or others.
  • Requires the ability to work after business hours, weekends, and holidays.
  • Requires the ability to utilize office equipment, including personal computers.
  • Requires the ability to travel in the performance of job duties.
  • Requires ability to pass the IDHS background check.
  • Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.

Work Hours: Monday-Friday 8am-4:30pm

Will be required to work one (1) Early shift choice of 6:00 AM- 2:30 PM or 6:30 AM- 3:00 PM per month Will be required to work one (1) Late Shift choice of 12:00PM- 8:30 PM, 12:30 PM-9:00 PM, 1:00 PM- 9:30 PM, 1:30-10:00 PM per month May be required to work one (1) additional early or late shift per month if needed Will be required to work one (1) weekend day per month
